Community Health Snapshot: What’s Working and
Where We Can Improve

Infographic titled “What’s Going Well” highlighting community health strengths. Breastfeeding rates exceed state goals and childhood immunization coverage meets the NYS 2030 target. Harm-reduction efforts are strong, with good access to naloxone and buprenorphine. Eighty-six percent of adults report being food-secure. Health indicators show fewer asthma ER visits among children, lower avoidable hospitalizations for adults, most adults having a regular healthcare provider, and adult physical activity levels close to the state benchmark.

Infographic titled “Areas for Improvement” summarizing community health concerns. Key issues include low early prenatal care rates and infant and maternal mortality more than double state goals; high levels of adult mental distress, suicide, and opioid prescribing; 24.3% of deaths occurring before age 65; low preventive dental visits among Medicaid enrollees; chronic absenteeism among K–8 students; poor fruit and vegetable consumption among adults; and social challenges including poverty, unemployment above target, and high rates of childhood trauma.
